Ownership authorization. I own the items listed above or have legal authority to authorize their removal.
Prohibited items. I confirm that no hazardous chemicals, fuel, wet paint, asbestos, explosives, ammunition, medical waste, biohazards, unidentified liquids, radioactive materials, or other regulated hazardous waste are included in the items to be removed.
Valuables removed. I have removed everything I want to keep and understand removed items may be disposed of, recycled, donated, resold, or salvaged and cannot be returned.
Access and safety. Pathways are reasonably clear, pets and children are secured, and known hazards have been disclosed.

Job documentation (required). I authorize the company to take before-and-after photographs of the items and work area for job records, quoting, and dispute resolution. These photos are kept in business records and are not published.
Marketing use (optional, separate). I separately and voluntarily permit the company to use job photographs in advertising, on its website, and on social media. This permission is optional, is not required to receive service, and may be declined or withdrawn.
